• Use hypnosis as a tool in treating depression, anxiety and other disorders

PAY IN INSTALMENTS FOR YOUR CONVENIENCE:

• When booking both workshops before January 31st at a total price of £770, pay £250 when submitting your form and the balance by 31/10/04 ,
• When booking both workshops before April 30th at a total price of £900, pay £350 when submitting your form and the balance by 31/01/05

Payment methods:

• by cheque: send two cheques when booking, one for the deposit amount, and one for the balance amount back-dated to the payment deadline,
• by credit card: fill in your credit card details. We will take a first payment for the deposit amount on receipt of your booking form and take a second payment for the balance amount on the stated deadline.

COURSE INFORMATION

• The workshops will be held at an accessible Central London venue to be confirmed
• Course fees include: all tuition with Dr Michael Yapko, course materials and lunch and coffee breaks every day. Accommodation is not included
  

ENTRY REQUIREMENTS
Applicants for the Introductory workshop need to:

1. either
   a).hold a minimum of a diploma or degree in a relevant healthcare or mental health discipline (eg. psychotherapy, counselling, nursing, psychology, clinical hypnosis, health care studies, etc.)
   or
   b).be currently at an advanced level/stage (eg. final year) of a recognised training programme (as in 1a.)
2. Have a current membership of a recognised professional body (eg. BPS, GHR, BACP, UKCP, BSCH, APT, NACHP, etc.)
3. Have up-to-date professional indemnity insurance

Have clinical experience with patients/clients
(All requirements 1 to 4 need to be fulfilled)

Applicants for the Intermediate/Advanced workshop need to fulfil the all of the above requirements, plus in addition the need to:

5. Have prior formal training in hypnosis
6. And have prior experience in performing hypnotic inductions and facilitating hypnotic phenomena (eg. in addition to fulfilling criteria 1-4, the applicant has successfully completed the Introductory Module of Dr. Yapko's training course, and/or has previously undertaken training in clinical hypnosis with a professional training organisation, and can perform hypnotic inductions and facilitate hypnotic phenomena).

All applicants should include with their application form photocopies of:

1. Their up-to-date indemnity insurance certificate
2. Evidence of their membership of a recognised professional body
3. The certificate of their most advanced qualification in a relevant healthcare discipline
